Central Health’s Eligibility Services team helps you and your family find and apply for programs to cover your health care needs. We also connect people to nonprofit health care providers in our community.

Our team will:

• Ask questions
• Review your supporting documents
• Determine if you or your family members are eligible for our Medical Access Program (MAP)
• Assist you and your family members in completing applications for some State of Texas health care programs
• Make referrals to health care providers as needed
• Answer your questions
